Small Claims FAQ Answer to: How can I find a Small Claims dismissal form?

The Request for Dismissal form can be used for small claims cases.

Also, you can request a more simplified local Dismissal Request from the Los Gatos, Palo Alto or South County courthouses, either in person (see courthouse location page) or by mail (see same courthouse location page for mailing addresses also) by sending your request along with a self-addressed stamped envelope.
