Delinquency FAQ Answer to: How can I get copies of my Juvenile file?
  
You can request the copies in person or by mail. The fee for copies is .50 per page. Certification is $15.00 per document. If requesting by mail, you may send in a written request with your signature notarized. In your request, provide your name and date of birth.

If you aren't sure the number of pages to be copied, you can enclose a check stating "Not to exceed $________" and we will fill in the correct amount and send you a receipt with your copies. Also enclose a self-addressed, stamped envelope.

In addition to the notarized letter, you will need to complete a "Petition for Disclosure of Juvenile Records" (JV-570) which is available on the state form website or by clicking this link: Form JV-570.

The exception to filing a Petition for Disclosure is limited to:

  • a person seeking copies for the purpose of providing required information to the Immigration and Naturalization Service for citizenship purposes, or

  • the United States Armed Forces, or

  • to obtain employment as reflected on the Declaration for Juvenile Records.

The petitioner shall receive a copy of the dismissal order or minute order only identifying the minor's name, date of birth, juvenile petition number and the date of the dismissal. The copy and certification fees still apply.
