page provides help in finding and using the legal forms needed at court.

Small Claims and Unlawful Detainer
(eviction)
forms. This service will help you complete your forms, but you will
still need to file them with the Court. This is not online filing!
If you know the form name or number
Forms are either state forms or local forms. Most forms used at
court are state forms and are on the state Judicial Council website.
State forms
To find forms on the state form website:
- From the dropdown menu you can choose "All Forms Listed by Number"
or "All Forms Listed by Name" or select the type of form you want
(such as Small Claims)
- Hit the "See Forms" button
- Scroll till you see your form
Click here for the state form page. Note that when you list the forms, there are two columns of numbers. If
you click on the form number in the first column you must print it to fill
it out. If you click on a number in the second column, you can fill it out
online and then print it.
If you have any problem at the state site, you should contact the
Judicial Council to report it.
Local Forms
If you don't find the form at the state site, check the local form page
to see if the form is there.
Click
for the local forms page. The forms are in
alphabetical order by form name. Start at the top of the form table and scroll through
the list looking in the third column, which displays form numbers. You can
also search the page (using control F) for name or number you are looking
for.

If you don't know the
form number or name
If you don't know the form number or name, you can read the section on
the topic relating to that form at one of the court self help sites. For
example, if the form relates to divorce, read over our
divorce page. If the form relates to a
name change for a child, read the
minor name change page
at this site. Check the site map for
the page you want.
On the Judicial Council site, you can use the dropdown menu on the
forms page to pick the topic the form relates to, such as Adoption or
Small Claims. Then click "See Forms" to see all state forms in that area.
Or you can try using the state's
Self Help
web pages.
Another alternative is to use the "EZFORM" program, a
court application that helps you complete the correct forms in a
question/answer format. This service will help you complete your
forms, but you will still need to file them with the Court. This is not
online filing! The service can be used for the following legal areas:

Guardianship Probate

Small Claims

Unlawful Detainer (eviction)
Note - problems at the EZFORM site should be
reported to San Mateo Superior Court, the agency that supports this
application.
If you still can't find the form
If you were told to use a form by someone at the court, check the form
name and number again by first rereading your paperwork, and then
contacting the court if necessary. Click for ways to
contact the court.
Not all matters placed before the court are made on pre-defined forms.
In some cases, the information will be
submitted to the Court using pleading paper.
If
you cannot find a form that applies to the matter you want to place before
the court, please refer to
CRC Rule 201 on how to prepare a pleading.
If you are still having trouble, you may want to speak with your
attorney. If you don't have an attorney, see the
local
attorney referral site. Or, visit our self
service center to ask
about the form.
If you experience a problem with the form after
locating
it
All forms are in PDF format and you must be on a computer that has
Adobe Acrobat reader. If you find a form but can't open it, be sure you
have the Acrobat software. (You can download and install a
free copy from
Adobe.) Try opening another PDF to see if the problem is for all
PDFs, or just this one form.If the form appears to be in error, or
have an incorrect revision date,
please email the court at
ssweb@scscourt.org and notify us of:
- the
form name
- the form number
- the URL/web address of the form itself (should end in ".pdf", such
as "http://www.courtinfo.ca.gov/forms/fillable/fl670.pdf")
- the URL/web
address of the page where you clicked on the form (should end in ".htm" or
".html", such as "http://www.scselfservice.org/fam/childsupport.htm")
- the error that you experienced while trying to access or use the
form
EZ Legal Form
Our court's partnership with San Mateo Superior Court allows us to
offer an online interactive form generator "EZ Legal Form"
which can be used for some Family, Small Claims, Unlawful Detainer
(housing), and Probate/Guardianship
forms.
A question/answer format will lead you through the application, which will
fill out the right forms for you. The forms can then be printed and submitted to
the Court. This service will help you complete your forms, but
you will still need to file them with the Court. This is not online
filing!
