FAQ Answer to: How do I find the Change of Address form?

The change of address form is a state form. You can access it and complete it online at http://www.courtinfo.ca.gov/forms/fillable/mc040.pdf.

Then you will need to print it out and mail it to the court where your case was heard - refer to the list of court mailing addresses.
