Family FAQ Answer to: How do I change the father's name listed on my child's birth certificate?

There are 2 ways this can be done.

The first way: file the state form Petition to Establish Parental Relationships (FL-200). Once you get the judgment of Paternity you can send a certified copy of it along with a VS22 form to Sacramento. The mailing address is:

Bureau of Vital Statistics Dept. of Health Services
304 "S "Street
P.O. Box 730241
Sacramento, CA 94244-0241

The second way: go to the Health Department on Moorpark Avenue in San Jose and fill out the local form they have for paternity. Both parents must be present for this process.
